Equitable Patient-Centered Care
Equitable Patient-Centered Care
EQUITABLE PATIENT-CENTERED CARE STATEMENT
I believe in caring for the whole person—body, mind, spirit, and environment—through a patient-centered approach grounded in compassion, presence, and respect for human dignity. My work in underserved communities has strengthened my ability to listen deeply, honor individual narratives, and remain attentive to the influence of cultural and social factors on health. I aim to build trust and therapeutic relationships that support patients in feeling safe, respected, and actively engaged in their care.
Guided by my passion for movement, nutrition, and nature, I support whole-person wellness while maintaining a commitment to humility, inclusivity, and continuous learning.
EQUITABLE CARE ACROSS POPULATIONS
CARDIOVASCULAR
GERIATRICS
SUBSTANCE USE DISORDER
Cardiovascular care focuses on supporting heart health through medication management, lifestyle changes, cardiac rehabilitation, and ongoing monitoring of blood pressure and cholesterol. Care is delivered in a safe, supportive, and trauma-informed way that emphasizes education, shared decision-making, and patient empowerment. This aligns with my mission by combining clinical excellence with compassionate guidance to help patients build sustainable, heart-healthy habits that fit their lives.
Geriatric care focuses on supporting older adults through medication review, fall prevention, cognitive and functional assessments, and management of multiple chronic conditions. Care is provided in a respectful, trauma-informed way that prioritizes safety, dignity, independence, and clear communication. This reflects my mission by integrating holistic healing and patient-centered support to create a sense of security, simplicity, and comfort while honoring each person’s goals and life experience.
Care for substance use disorder includes evidence-based treatment such as medication support, withdrawal management, counseling referrals, and relapse prevention strategies. Care is delivered in a nonjudgmental, trauma-informed way that prioritizes trust, emotional safety, and patient autonomy throughout recovery. This aligns with my mission by combining clinical care with empathy and holistic support, empowering individuals to heal while addressing the deeper physical, emotional, and social factors that influence substance use.
“In everyone’s life, at some time, our inner fire goes out.
It is then burst into flame by an encounter with another human being.
We should all be thankful for those people who rekindle the inner spirit.” -Albert Schweitzer
